The following paperwork will need to be completed and returned to the Troop Committee room with your registration/fees.  Make checks payable to “Troop 146, BSA”.

  • Troop 146 Registration Form (one form per Troop member)
  • BSA Youth Application
  • BSA Adult Application for parents/guardians planning on volunteering with the Troop
  • Bethany Boy Scout Troop 146 Code of Conduct & Discipline Policy - both Scout and Parent must read and sign
  • BSA Medical Form Parts A & B
    • Part C requires a physical and doctor signature and is only needed for scouting events over 72 hours
  • Adult Volunteer Opportunities


NOTE:  All of these forms will be provided in an envelope once the scout and family have selected Troop 146
